Course Overview

The Level 3 Community Health and Wellbeing Worker Apprenticeship prepares learners to work in partnership with individuals, groups and communities to improve health and wellbeing, prevent ill-health and reduce health inequalities. Community Health and Wellbeing Workers may operate across the NHS, local government, voluntary, community and social enterprise organisations and other community-based services. The programme has a 12-month training duration, with approximately 15 months including End-Point Assessment (EPA).

The programme develops learners’ understanding of the wider determinants of health and the social, cultural, economic, environmental and behavioural factors influencing physical, mental, emotional and social wellbeing. Learners develop holistic, person-centred and strengths-based approaches that enable them to identify needs, recognise individual and community assets and address barriers to improved health and wellbeing.

Across the programme, learners develop skills in community engagement and empowerment, partnership working, service navigation and referrals, behaviour change, health improvement, communication and health promotion, governance and ethical practice. They learn to work collaboratively across services, support sustainable behaviour change, communicate public health information and operate safely within safeguarding, information governance and professional boundaries.

The planned curriculum provides a structured journey through the occupational duties. Initial learning includes induction, self-assessment and EPA awareness, followed by personal and professional development and legal and ethical frameworks.

Topics may include:

  • Health, wellbeing and ill health
  • Behavioural science
  • Providing informed advice
  • Managing referrals
  • Communicating health messages
  • Promoting wellbeing and community resilience
  • Implementing health and wellbeing strategies
  • Using data and information.

Personal and professional development is revisited throughout, supporting reflection and the application of learning to workplace practice.

Assessment Methods

Assessment is designed to provide valid and reliable evidence of occupational competence. The assessment plan requires at least one professional discussion, alongside additional methods which may include a portfolio, case study, question and answers, journal, simulation, observation or additional professional discussion.

Learners are assessed against seven overarching assessment outcomes, with performance considered across applied knowledge and skills, regulatory and procedural awareness, communication and collaboration, information use and decision-making, and responsibility and autonomy.

Assessment differentiates between Pass and Distinction, with higher performance demonstrating greater independence, judgement, initiative and effective application of learning within community health and wellbeing practice.

Progression Options

On successful completion, learners may progress into roles such as Community Connector, Health Trainer, Live Well Coach, Social Prescribing Link Worker or Care/Service Navigator.

Further progression may include senior or specialist community health, health improvement, public health, coordination or supervisory roles. Learners may also progress to relevant Level 4, Level 5 or higher education programmes in health, public health, health and social care or community development.
